Comment Card Procedures

I.
Comment Cards at Public Service Desks
- Comment cards should be
removed daily from all public service desks.
- The date and location where it was received should be written
on the back.
- Forward all comment cards to the Public Services Administrative
Assistant, LL 536.
- Additional
comment cards are available from the Design Services Offices.
- The Public Services Administrative Assistant will be
responsible for storing the hard copy
of the original
comment cards.

II.
Electronic Comments
-
If the Comments have been received electronically, outside of the
Comment Card database, they will
be forwarded to the Public Services Administrative
Assistant (kjhill@uci.edu),
who will input them into the
Comment Card database.
- Comments received through the UCI Libraries website will automatically be placed
in the
Comment Card database.

III. Distribution of Comment Cards and Electronic
Comments
-
The Public Services Administrative Assistant will review the comment
cards, input them into the
Comment Card database, and forward them to the appropriate
person for a response within 5 working days..
- All comments entered into the Comment Card database either
by the Public Services Administrative Assistant, or
through the UCI Libraries Website, will be automatically
forwarded
to:

Services Administrative Assistant,

IV. Receiving a Comment for Response
-
If you personally receive a comment that includes an email address,
please respond to the user within 5 working days
(including the text of the original question) and copy
the Public Services Administrative
Assistant (kjhill@uci.edu),
who will
input the comment and response into the Comment
Card database.
- If you receive
a comment with no email address but you feel a response to the particular
comment would
benefit other users, please email a response to the Public Services
Administrative Assistant (kjhill@uci.edu),
who will input the comment into the Comment Card database.
- Web Services will keep a database of all of the comments.

V. Quarterly Comment Survey
- At the end of each quarter, the Public Services
Administrative Assistant
will create a summary of all
comments received that quarter and provide it to the Associate University
Librarian for Public Services and the UCI Libraries
Director of Planning, Assessment and Research.
The
summary
will
include
the complete text of the comments, the
location where the comment
was received, the date it was received, who the comment
was
forwarded
to and, if applicable, a summary of
the response that was provided.
- The AUL for Public Services will share the summary as needed.
